Supplemental Purse Award - File this form within 15 days of your horse's win at a WV racetrack to be eligible to receive up to ten percent in breeders awards.

Authorization to Release Information - By filing this form, you allow the WVRC to review your annual West Virginia personal tax returns. Leave the "authorization terminates" line 2 blank and it can be used for all years. Nonetheless, copies of receipts for real estate taxes paid, personal property taxes paid, or rental leases must be submitted annually as additional proof of residency.
